no you don't have to pay for it monthly if your just ganna play single player but ive heard the online mode is well worth the charge. I have the game but i havn't custumized my character yet anyone know when you get to do that?

You can customize them offline. In the actual single player you can buy new clothes for Ethan.
When you get to Chapter 4 you unlock "Extra Mode" which is like an offline version of the online game, where you can make a new character and customize them etc.
